The PIE Committee approved the attached Civil Indigence form at their June 13, 2018 meeting. It was then approved by the Executive Council at their June 25, 2018 meeting. In accordance to what was presented at the Executive Council meeting, the attached form is to be emailed to [email protected] by close of business on August 20, 2018 and will be due monthly thereafter.
